CHICAGO - Jose A. Lezama, age 60 of Chicago, passed away on Thursday, July 26, 2012 at the University of Wisconsin Hospital in Madison. A memorial mass was held in Chicago on Aug. 3, 2012 prior to his cremated remains being transported to Alvaro Obregon, Michoacan, Mexico for burial.
